Royal Institute of British Architects (Sixth Floor), 66 Portland Place, London W1.

Underground: Regent’s Park Station, Great Portland Street, Oxford Circus.  Parking: Carburton Street (NCP).

Who should attend?

This meeting is suitable for all those undergoing training (or those considering training) in either neonatology or obstetrics.  Delegates may book individual surgeries during lunchtime for advice on career pathways and/or information on specific training requirements.

Fees and payment

The registration fee is £95 (including lunch and refreshments) on all registrations received on or before 18 October 2008 (from 19 October onwards, the registration fee will be £105). 
You may register online but we do not have facilities to take payments via the website.  Payments can be made by cheque or credit card.  Payments must be in sterling – other currencies cannot be accepted.  Confirmation of your registration can only be sent when payment has been received.  If you  have not received confirmation of your registration within 14 days of sending your payment, please contact us to confirm that we have received your booking form. Please note that you are not guaranteed a place until you receive this confirmation. If applying after 18 October 2008, you should telephone 020 7092 6085 to confirm that a place is available before posting your application form and payment.

Cancellations

Cancellations should be confirmed in writing or by email. An administrative charge of £10 will be made on all cancellations received before or on 18 October 2008. BAPM cannot refund fees after this date. Cancellations will be processed after the conference.

CPD

You may claim one CPD point per hour (excluding lunch). Certificates of attendance will be provided.

The details you provide on this form will be held and processed electronically. Your name, hospital and email address will be included on the list of participants that will be issued to delegates, speakers, sponsoring companies and other interested parties.
