The Annual Dinner will be held on the evening of Thursday 10 September 2009. Who should attend?The Policy Development session and Founders Lecture (on 10 September) and the continuation of the Scientific Day (on 11 September) are open to all those involved in care of the pregnant mother, fetus and newborn ie obstetricians and neonatologists, nurses, midwives and other health professionals. The AGM is restricted to BAPM members and invited guests. Transport & AccommodationLeeds has very good transport links from all over the UK whether travelling by air, road or rail. Accommodation is not included in the registration fee but some rooms have been reserved at a 'corporate' rate at the following hotels: Please note that these rooms will be allocated on a 'first come, first served basis' so delegates wishing to book accommodation in these hotels are advised to do so as soon as possible. Fees and paymentThe registration fee includes lunch, refreshments and access to the exhibition for one person. 2 days (no accommodation) - £155 You may register online but we do not have facilities to take payments via the website. Payments can be made by cheque or credit card. Payments must be in sterling - other currencies cannot be accepted. Confirmation of your registration can only be sent when payment has been received. If you have not received confirmation of your registration within 14 days of sending your payment, please contact us to confirm that we have received your booking form. Please note that you are not guaranteed a place until you receive this confirmation. If applying after 28 August 2009, you should telephone 020 7092 6085 to confirm that a place is available before posting your application form and payment. CPDYou may claim one CPD point per hour (excluding lunch). Certificates of attendance will be provided. CancellationsShould be confirmed in writing or by email. An administrative charge of £20 will be made on all cancellations received in writing before or on 25 July 2009. BAPM regrets that we cannot refund fees after this date. Cancellations will be processed after the conference. Conference DinnerAll delegates are invited to attend the Annual Dinner on Thursday evening, 10 September. The charge for the dinner is £55 per person and includes a drinks reception and entertainment. Data Protection ActThe details you provide on this form will be held and processed electronically. Your name, hospital and email address will be included on the list of participants that will be issued to delegates, speakers, sponsoring companies and other interested parties.
